Abstract:

Background and Objectives: Complete HYDATIDIFORM MOLE is the most common form of trophoblastic disease, the incidence of which greatly differs geographically and is fairly high in our country too. The aim of this study was to determine the frequency of molar pregnancy in our patients and assess the ultrasonic findings of the disease.Materials and Methods: From July 1985 to February 1997, 11,370 women with normal or pathologic pregnancies underwent ultrasonic examination in Obstetrics and Gynecologic (Al-Zahra) Hospital of Faculty of Medicine, Tabriz University of Medical Sciences and Azarbaijan Radiologic and Sonologic Clinics (Private). Results: Among 11,370 patients, 10,414 women had normal pregnancies and from 935 remainders with different associated pathologic conditions, 100 patients had HYDATIDIFORM MOLE (98 complete and 2 partial MOLE). In this study, the incidence of HYDATIDIFORM MOLE was 8.7 in 1000 pregnancies which approximate to Saleh and co-worker's study (1964-65), but has considerable differences in comparison to Javey and Sajjady’s findings (1970-75) and the Western and the Far East countries statistics. Conclusion: Occurrence of hydatiform MOLE is not a common finding, however its prevalence is fairly high in our country in comparison to other studies reported earlier.

A 23-year-old Gravida 3 Para 1+1 at 26 weeks and four days of amenorrhea presented at our obstetrics and gynecology clinic for assessment following a coincidental scan that revealed a single live intrauterine fetus in cephalic presentation, longitudinal lie with normal cardiac activity, heart rate of 150 beats per minute, normal gross anatomy and estimated fetal weight of 992 grams. . .

Abstract:

Background and Objective: Gestational trophoblastic disease refers to a specific range of trophoblast proliferative disorders that originate from the placental epithelium. Recently, hematological indices are being researched and discussed as an easy and accessible method to identify patients and monitor disease progression. Therefore, the present study was conducted to investigate the relationship between the levels of hematological indicators and HYDATIDIFORM MOLE progression. Methods: This cross-sectional study was conducted on 164 patients diagnosed with HYDATIDIFORM MOLE in two groups: complete HYDATIDIFORM MOLE (132 people) and partial HYDATIDIFORM MOLE (32 people). The diagnosis of the disease was made based on the pathology results recorded in the patients' files. Information related to hematological, biochemical and hormonal indices was collected and analyzed. Findings: In general, there was no statistically significant difference between the group of complete HYDATIDIFORM MOLE and partial MOLE in terms of age, body mass index, uterine size, incidence rate of Theca lutein cyst and cyst size, gravida, parity, miscarriage, and history of previous MOLE. Of all laboratory markers evaluated (such as WBC, PLT, MPV/PLT, AST/PLT, RDW/PLT), only the mean level of triiodothyronine (free T3) in the complete MOLE group (1.50±2.40 pmol/L) was significantly lower compared to the partial MOLE group (1.90±1.30 pmol/L) (p=0.004). Free T3 threshold (1.54±0.87 pmol/L) showed good sensitivity (74.60%), specificity (51.50%) and 50% accuracy to distinguish complete MOLEs from others. Conclusion: This study showed that a high level of free T3 may be a good marker for the diagnosis of HYDATIDIFORM MOLE, which may be useful in the early detection of MOLEs undetectable on ultrasound.

Abstract:

HYDATIDIFORM MOLE with a coexistent fetus is a rare occurrence with an incidence of 1 per 22000-100000 pregnancies. It is associated with persistant gestational trophoblastic tumor. Hence an early and correct diagnosis is imperative to plan subsequent management of such patients. We report a case of primigravida who presented with vaginal bleeding and severe nausae and vomiting. Expectant management was carried out for her pregnancy with finally ended in abortion.

Abstract:

Tubal pregnancies are common but presence of HYDATIDIFORM MOLE in tube is a rare entity. A woman of 53years presented with acute abdomen, preoperative pregnancy test was positive and ultrasonography suspected the case as HYDATIDIFORM MOLE in right tube. The patient was treated with total abdominal hysterectomy with bilateral salpingo-oophorectomy as her family was completed. Subsequent histopathological examination showed ruptured complete HYDATIDIFORM MOLE in right tube. Postoperative period was uneventful with normalization of b-HCG titer within two weeks.
